Assessment
At Buenavista SLS, we offer skills assessments to get a clear picture of what each person needs. Our Program Director will meet with the individual, either at home or in the community, to understand their unique needs. This helps us create a personalized plan or develop new services just for them. For those in our Supported Living Programs, we use these assessments to tailor our services, working closely with the regional center to make sure everything fits your needs perfectly.
Goal Setting
The Individual Support Plan (ISP) is a foundational element in Buenavista SLS’s services, crucial for delivering personalized, effective, and responsive care to individuals. It ensures support is tailored to each Client’s unique needs, goals, and preferences, enhancing their overall quality of life. The ISP facilitates goal setting and achievement by outlining clear, achievable objectives and steps to reach them, helping individuals improve daily living skills, secure employment, or strengthen social connections. Moreover, goals that are set will be created in collaboration with the individual and will be focused around objectives that are “important to them” as opposed to what is “important for them”.
Care Plan
Supported living services create a comprehensive care plan that addresses various aspects of a client's life. The plan focuses on enhancing relationships by developing social skills and building a strong support network. It includes managing home life, such as organizing the living space, planning meals, and providing personal care. Health and safety are prioritized with oversight of medical needs, personal hygiene, and home safety. This holistic approach ensures clients receive personalized support to lead an independent and fulfilling life.
Support for Medically Fragile
Buenavista SLS stands out with our dedicated team of medical staff (RN, CNA, LVNs), ready to provide extra support where it’s needed most. Whether individuals need help with oxygen delivery and monitoring, BIPAP, or specialized care for gastrostomy and ileostomy, we’ve got you covered. Our skilled nurses are also here for managing catheters, tube feeding, medications, pressure sores, diabetes, and more. From addressing obesity and seizure disorders to other specific nursing needs, we’re here to offer compassionate and comprehensive care tailored to you.
